Web Application experiencing intermittent System.AccessViolationException

I have a web application (vb.NET) that is experiencing intermittent (one out of every 5 or so attempts) System.AccessViolationException on the client web servers (Windows Server 2008 R2 and also a new Windows Server 2016). Cannot recreate this on my development machine or on our testing web server (2008 R2). The error seems to occur on accessing the Oracle database (using OracleDataAdapter) in a worker thread. When I took the threading out, the application did not throw the error but response time is much lower.
