Create text box and write in a pdf

I use this code to display the pdf document on the screen, so far it works fine:

After opening the pdf, I wanted to be able to create a text box so that I could write in the pdf and save those changes with the text box in the pdf itself, as I show in the image below:

I do not create the pdfs in php, I upload the pdfs to a folder inside the server and save the path in the database table.
Can anyone help create these text boxes?
