Index and match with multiple criteria and newest date

Sheet 1 I have a name and date, I am trying to return a number from sheet 2. Sheet 2 I have list of names (they can appear more than once) a start date, a end date and entered date and the unique number to that line.

Sheet 1 name has to match sheet 2 name
Sheet 1 date has to be after or equal to start date and before or equal to end date.
I need to then locate the newest entered date and return the ID.

You may want to take some time to read this and this, then please come back and edit your question to let us know what you've tried so far and how it fails to meet expectations. It would also be useful to have a screencap of both sheets.
