If the answers to any of the 3 questions above are left blank, the presumed answers for those questions will be no.

Asking about "answers are" vs "answer is" I could understand, but since the verb is plural in both cases, your question really answers it self. "Answer are" is not English.

The first one is correct. But the second one would be correct too, except that you forgot to make the rest of the sentence agree with the singularanswer. It should be: "If the answer to any of the 3 questions above is left blank, the presumed answer for that question will be no."
